My memory, from having worked on the Landsat
satellite system, is that there are a large
number of "multiplexing" algorithms.  For 
example, I know the designer of the original
Telebit Trailblazer, which uses a frequency
division scheme, and it is still the best
overall modem for communicating with less
developed countries, where phone networks are
poor quality, at best.  The modem continuously
samples the transmission quality in 12-15 
"bands," and adjusts dynamically to changes
in the fidelity of any given band.  Sadly,
this approach did not become the dominant
technology in America.
